Overview

What it is

Therapeutic Plasma Exchange (TPE) filters toxins, inflammatory molecules, and harmful antibodies from your plasma. Every session at Apeiron Center runs on the Terumo Spectra Optia, with three volume options: low volume (1 liter), a gentle, tolerance-first session; and full volume at 1.0 or 1.5 plasma volumes, a full therapeutic exchange.

Why it matters

Plasma exchange can reduce circulating inflammatory mediators, antibodies, immune complexes, and some plasma-bound substances. Volume sets the dose: a 1-liter session delivers a measured reduction (about a quarter to a third of circulating plasma-phase targets) with a lower physiologic burden, while a full exchange of 1.0 to 1.5 plasma volumes removes roughly 63–78%. We help you choose the right volume for your goals, biology, and tolerance.

02 · The math of exchange

Why volume matters, and what it means for your body.

Low volume · 1 liter~25–33%

Of circulating plasma-phase substances removed per session.

Full volume · 1.0 plasma volume~63%

Of circulating plasma-phase substances removed per session.

Full volume · 1.5 plasma volumes~78%

Of circulating plasma-phase substances removed per session.

The curve of diminishing returns.

Think of your plasma like a glass of diluted tea. The first pour of clean water dilutes it dramatically. The second pour dilutes it less. The third, less still.

Moving from nothing to one full plasma volume produces the largest drop in circulating substances. Each additional fraction removes progressively less, so the dose isn’t just a number; it’s a curve.

This is why a 1-liter session is not simply “half” of a full exchange. It’s a meaningfully different intervention with different biological consequences: gentler, yes, but also narrower in what it can accomplish clinically.

Figures assume a one-compartment intravascular target and reflect the classical continuous-flow exchange model. Real-world outcomes depend on molecular size, tissue redistribution, and resynthesis rates.

03 · Which option is yours

Your goal determines the dose.

  • Full volume preferred

    Preventive longevity

Published biological-age studies used one full plasma volume with 5% albumin, sometimes combined with low-dose IVIG. A 1.0 plasma-volume exchange delivers the protocol studied in the research.

  • Full volume preferred

    PFAS and forever chemicals

Full-volume exchange removes a substantially greater fraction of circulating protein-bound PFAS per session than low-volume plasma removal, delivering deeper burden reduction.

  • Full volume preferred

    Heavy metals

Chelation remains primary therapy. When plasma exchange is used as an adjunct, full-dose exchange is mechanistically aligned with maximal circulating removal in a single session.

  • Full volume preferred

    Microplastics and nanoplastics

Evidence is emerging and investigational. When exchange is pursued, a full-volume exchange offers the greatest per-session reduction of circulating particles.

  • Full volume required

    Autoimmune or inflammatory disease

Conditions with high antibody burden, hyperviscosity, cryoglobulins, or ASFA-recognized indications require the full therapeutic dose. Fractional removal is likely to under-treat these mechanisms.

  • Full volume required

    Hyperviscosity, cryoglobulins, paraproteins

These are classic intravascular plasma-protein problems where rapid plasma removal is mechanistically aligned. Only full exchange produces the reduction these conditions require.

  • Low volume preferred

    Frail, sensitive, or low-reserve

Lower body weight, dysautonomia, chemical sensitivity, older age, or a history of procedure intolerance all favor the gentler option. The smaller perturbation reduces risks of fatigue, calcium symptoms, or hemodynamic stress.

  • Low volume preferred

    Tolerance assessment

If you're considering a future full exchange but want to understand how your body responds first, the 1-liter option is an ideal proving ground: we can assess vascular access, citrate tolerance, and post-procedure recovery before escalating.

  • Low volume preferred

    Maintenance after an initial exchange

Some patients pair one or two full-volume exchanges with a maintenance schedule of low-volume sessions. This staged approach preserves the initial reset while minimizing cumulative depletion.

  • Case by case · often full volume

    Severe refractory neuroimmune Lyme

Plasma exchange is not antimicrobial therapy. It is occasionally used as rescue therapy for severe presentations that resemble GBS or autoimmune neuropathy. Antibiotics remain central.

  • Low volume if mild · full volume if severe

    Mold illness and CIRS-like syndromes

Evidence for mycotoxin removal by plasma exchange is weak. Framed honestly, TPE may reduce some circulating inflammatory mediators, but source control, exposure cessation, and downstream support remain foundational.

  • Discuss with your clinician

    Not sure where you fit

Your history, goals, biomarkers, and tolerance for physiologic change all inform the right starting point. This is exactly the conversation we begin in consultation.

04 · Patient profiles

Two approaches, each with its own rhythm.

Low volume · 1 liter

The measured optimizer

Proportional, iterative, and tolerance-first.

  • Values gradual change over dramatic intervention.
  • Prefers lower fluid, electrolyte, and protein perturbation.
  • Is exploring plasma exchange cautiously for the first time.
  • Is moving from a full exchange into a maintenance rhythm.
  • Is frail, dysautonomic, chemically sensitive, or recovering reserve.
  • Wants to understand how their body responds before escalating.

Full volume · 1.0 or 1.5 plasma volumes

The comprehensive reset

Full-dose, evidence-aligned, research-grade.

  • Wants the intervention that matches published longevity trials directly.
  • Is pursuing the deepest available per-session reduction of PFAS or heavy-metal burden.
  • Has a recognized autoimmune or inflammatory indication where dose matters.
  • Presents with high antibody, immune-complex, or inflammatory burden.
  • Has hyperviscosity, cryoglobulinemia, or paraprotein disease.
  • Is prepared for closer monitoring of calcium, proteins, and coagulation.
  • Is comfortable with the greater physiologic perturbation of a full exchange.

How we frame the science

Evidence, honestly framed

Plasmapheresis vs. plasma exchange: the distinction matters.

These terms are often used interchangeably, but they are not identical. Plasmapheresis describes the process of obtaining plasma from blood. Plasma exchange specifically means removing plasma and replacing it with a substitute fluid. Every option at our center runs on the Spectra Optia, a platform indicated for therapeutic plasma exchange; the volume you choose sets the dose.

On longevity claims: honest framing.

Early human studies using 1-plasma-volume exchange with 5% albumin have shown encouraging shifts in biological-age biomarkers and inflammatory markers. These are real signals worth discussing, but the evidence remains biomarker-focused, small-scale, and early. We present plasma exchange as a health-optimization intervention with evolving evidence, not a proven anti-aging treatment.

On PFAS, heavy metals, and environmental burden.

Plasma exchange can reduce circulating inflammatory mediators, antibodies, immune complexes, and some plasma-bound substances. For PFAS specifically, full-volume exchange produces a meaningfully larger per-session reduction than low-volume plasma removal. For heavy metals, chelation remains primary; plasma exchange serves as an adjunct. Benefit depends on whether the relevant burden is actually in the plasma at the time of the procedure, and whether the source has been addressed.

On the right first choice.

Our decision rule is simple. When therapeutic-dose mediator removal matters most, including longevity protocols, PFAS reduction, heavy-metal adjunctive care, microplastics research, and recognized disease indications, we recommend a full-volume exchange. When proportionality and tolerability matter most, for sensitive patients, tolerance assessment, and maintenance schedules, we begin with low volume. The choice is not a preference. It is a clinical match between your goals and the dose curve.

On monitoring.

Every procedure at Apeiron Center is accompanied by thorough baseline screening: CBC, comprehensive metabolic panel, albumin, total protein, fibrinogen, coagulation studies, ionized calcium, immunoglobulins where serial procedures are planned, and condition-specific markers. Full-volume exchanges also include session-specific hemodynamic and calcium monitoring.

A note on scope

The Spectra Optia is indicated for therapeutic plasma exchange. Applications beyond FDA-cleared indications, including longevity optimization, environmental toxin reduction, and off-label use for complex chronic conditions, are discussed with each patient as investigational or off-label where appropriate. Informed consent is central to everything we do.
